That is surprisingly (to me) ungenerous. Do most folks get 100% free from their employer or is that indicative of health insurance costs in Japan?

Most people are on employer-provided insurance, not the national insurance plan (covers principally retirees, unemployed, self-employed). EHI generally also has 30% co-insurance. Some companies may add additional benefits which decrease cash out-of-pocket for employees.

Important clarification would help — what happens to “major surgery” type of bills? Like, ¥10M? Are you bankrupt, or there’s an out of pocket max of some sort?
