My favorite part about being a cash payer in the US medical system is that ~6 months after a service you can get a bill in the mail with all numbers on it, inclusive of previous payments you've made, just made up. It keeps me on my toes.

Yes, especially when such processes are likely to violate something in Fair Debt Collection Practices Act, which they really would not want to do. PS. It's equally horrendous even if you have insurance, one hospital visit and you have a stack of copays from random people.
