Well, OK, my favorite plan is a Japanese-style system with single payer that has high deductibles for everyone except poor and old people, and a supplemental private insurance market, but I'm pretty sure no one is going to go for that, so.

Japan is not at all a single payer. It also has NO deductible. It instead has high co-insurance. Deductibles as a thing make basically zero policy/behavior sense compared to other cost-sharing like co-pays and co-insurance

Endorsing the accuracy of this comment, as someone who has been on Japanese national insurance (国民健康保険), arranged insurance for his company, and been on a company's insurance.
