RBS bank (huge global bank) was effectively offline for 6 days due to a botched upgrade. https://www.theguardian.com/technology/2012/jun/25/how-natwest-it-meltdown … This happened after years of forcing earlier retirement on their most expensive mainframe technologists and “offshoring” the work. No one was left who knew the system

Eventually they had to do a “deep reset” and put everything back as it was. That was the only way they could back online. Same issue happened again in 2016. Same issue hit Barclays later but was more isolated due to their business structure.
