Things I’ve heard at different places which attempt to correct for this: “We have never fired anyone for placing a phone call [including to the CEO]” and “We will under no circumstances negatively consequence someone for [pushing the Big Red Button].”https://twitter.com/kevinriggle/status/1072766424009994241 …

    Kevin Riggle @kevinriggle
I'll even expand this statement to be—"I screwed up, but the disincentives which are intended to discourage me from screwing up in the first place have compounded over time to the point where it's insurmountably painful to correct my mistake" is an antipattern. Cf "human error" https://twitter.com/kevinriggle/status/1072765402080403456 …

Fun fact: while I have been describing andon cords as “Big Red Button” for years because otherwise it’s a long story, I was amused to discover at Stripe that that is the literal proper name for the system.

A long while back I hit the proverbial red button on trading systems during a treasury auction. I was surprised I wasn't immediately let go. In retrospect, that was a good org.https://www.investopedia.com/terms/b/bill-auction.asp …

I did get to tell my tale to a rather stormy conference room with a bunch of partners. IIRC, the CFO was dialed in as well. They were pissed that I hadn't hit the button sooner.

Someone also asked why a mortgage guy was manning the treasury desk tech stack that day. I left the explanation of that bit of mismanagement to someone else.

Apparently at Elon Musk's companies utilizing his "open door policy" is a great way to get "managed out". I thought the bit here about the stop-the-line policy at Waymo was super-interesting thoughhttps://www.theinformation.com/articles/how-an-uber-whistleblower-tried-to-stop-self-driving-car-disaster …

I assume this is the reality in any organization of meaningful size, at least as it pertains to issues on your own team. Going outside the normal reporting chain is effectively saying "I don't think this will be resolved properly if I stay within the normal reporting chain."

Performance evaluations are largely subjective, so calling out the person who evaluates you, even in the most blameless/neutral way, is a risky move.

Reminds me of the time when, after a production incident that could have had rather auditable consequences, my CTO gathered us for a fireside chat and set the tone by describing the time he blew away the production database Also, "consequence" as a transitive verb? Yikes
