If an engineer accidentally deletes production data, should they be fired or was it more likely a failure of process? Assume small team, backup available, but customers were upset due to downtime.

Briefly: no company which would fire an engineer here (absent malfeasance) deserves either customers or employees. It’s entirely a process failure, and should be a learning experience for all concerned.

If you fire an engineer for downtime customers are upset about then you should also fire them for picking AWS which caused downtime customers are upset about, which _is madness_. Company / engineering management’s job is to manage risks; making decisions with downside is OK!
