Very nice summary. But it's interesting that there's been little attention to epidemiology data. Genetic data are troubling, but seems important to examine patterns of exposures and events for further insights. I'm sure this is being done, but haven't seen it presented yet.

Yes, I know of quite a lot of epi work but I don't think it has yet been published in detail, I hope soon!
