He wasn't on law review, although my recollection is that a majority of people who were on law review got on because of their score on the law review competition without considering their grades.

If Gorsuch were truly at the top of his class, as the president* claims, he would have graded on.

Likely, but not necessarily. One could be #1 in the class and not have taken the competition. Or (in theory) could have not done well enough on the competition to grade on 1L year but then have been #1 2L and 3L years.

Trumpspeak is at a different level of generality than most professionals. To him "first in his class" or "top of his class" means "super smart". That's the way a NYC construction worker might say it. He's talked that way all his life and he's not going to change.
