Great feedback! I do ask for name and email (optionally) as we are trying to build more personal relationships with our customers. I fear if I add that question no one will want to have these one on one conversations. Thoughts?

It's a little odd to have a survey ask how I used it and ask for my email. It sorta looks like it's more about harvesting my contact info than getting feedback.
