hey to be fair, i've seen some pretty good medical papers where having a control group would have been itself unethical...
Historically, when this argument is used it is often just a matter of years until someone figures out that the treatment isn't better than a placebo. E.g. arthroscopic surgery for osteoarthritis. Took years to convince people that sham (placebo) surgery is a necessary instrument.

(..and even then you sometimes have to wait for a generation change in doctors! Moseley et al published in 2002. My granny got arthroscopic knee surgery for osteoarthritis two years ago.
