No doc will ever just listen to your life story and then go "y'know, I think you might be trans, better get that tested". If you had to go through any form of gate keeping for trans medical care then I'm sure you'll agree on how far from reality that scene would be.

After a suicide attempt in 2005 I even told a psychiatrist "if I had the choice I'd rather live as women, but I don't feel like I have that choice and I don't think my dysphoria is bad enough to justify going through the troubles of transitioning." 3/4
