Pro tip: avoid as much as possible saying "idiot" and "crazy" in same breath as describing patients with neurological damage.

But to be clear, I wouldn't use those words even if I hate somebody. Just like I wouldn't say racist or sexist words.

I hear you. Sometimes I say these words when I am frustrated with someone. Has nothing to do with race or anything else. Just a manifestation of my impatience I guess. But that's just me.
