The DOMS in my arms/back — after skipping a good arms day for a few weeks and then doing some serious stuff — is the worst ever ever.

They love doms as a pain protocol in Sport and Health Science disciplines. Huge pain but no real damage

Fair. Is it even necessary for muscule damage? Or just sufficient?

It's weirdly uncorrelated with the amount of exercise or the muscle damage I gather (pretty new to me)
