As an editor, should I always be avoiding reviewers who the authors have explicitly listed as opposed reviewers on their submission?

Yes, unless there is some over-riding reason why they should review it, which is really rare.

Does it depend on the reason though? I had one cover letter list about 5-6 exclusions because they were "competing researchers".

I've had 2 people personally abuse me because ReAsOnS and I think I might have asked them to be excluded once, but now I'm less worried.
