Maybe this is because most people have little familiarity with anatomical diagrams, not because they don't know where and what vaginas are?

I would be tempted to agree but I recall conversations about violin plots (twice at least!) where quite a few psych & neuro ppl were saying

Another shock was 11 years ago when I first emigrated/moved to the UK and I discovered that people call vulvas vaginas.

Anyway, violin plots look like vulvas — while shark brains look like vaginas (plus more, fallopian tubes, etc).

My answer is many given my experience with English-speakers. In my other language we don't have this problem.
