I think a lot of these people are just oblivious. Also I don't think of my own relationship with the supervisor from a reference perspective

A student disagreeing with me (politely anyway) may sometimes be frustrating at the time - but it will certainly reflect well in their ref.

Biggest problem is that this disadvantages shy people. The confidence to challenge doesn't come naturally to everyone but can be learned.
