If they are at the conference itself or in their hotel they will have WiFi though? Also phones, talking on the phone, is way more anxiety-inducing than using a website with a form, right?

As is writing an email. Staring at a black screen and thinking about how to address it "Hi Organisers" etc. I think a web-form is way more likely to yield responses.

I have no tried-and-tested ideas here; at
@PyConUK I believe we speak directly from the reporting individual. Web form that’s not a text box would be good for getting key info. When? Where? What? Who? Specific Qs => more detailed, more actionable report.1 reply 1 retweet 1 like -

You can have an optional free form reply textbox too IMHO but I think it has to be optional.
