Well, I don't think that hierarchy is inherently oppressive but I can totally imagine world in which the existence of differences in knowledge and skills are appreciated without a fixed hierarchy. Why do you need to hierarchy to teach adults, which needs to be fixed in titles?

Agreed. In the Netherlands there is a habit of writing “Mrs. Dr.” in formal letters, which I dislike personally. My gender is / should be irrelevant for my professional title.
