The WHOLE SYSTEM IS BROKEN. It's not anyone parts fault. I work the phones at an insurance company -

You know that sometimes the emergency room charges HUNDRED OF DOLLARS FOR A BAND-AID, one like you buy at a drug store. -

Sometimes doctor's charge $1000 for physicals and hospitals charges hundreds for basic lab tests -

Everyone is trying to grab more of the pie and everyone is at fault. Doctors and hospitals try to take more out of companies as well -

Insurance needs to be fixed for sure it's gotten out of control as well but so has the general cost of healthcare. It's all broken.
