Well, unfortunately 90% of software developers, even if they know this, don't know how to do this. And when the system collapses, they rewrite it again, doing the same mistakes, with false feeling of implementing it better this time.

ALWAYS refactor, especially if it is before a rewrite. Nothing worse than attempting to rebuild without understanding how it broke I'm the first place. That is how to waste millions of dollars. I've seen it happen.

A+. I cannot stress this enough. I've been ranting about this to my coworkers for several years... Sadly, lots of software folks don't get this. They plow forward with top-down rules and centralized plans for utopia. The world does not work like that.. Decoupling is essential.
