From what I've seen and heard most don't even need to be hacked. They just leave security functions turned off and others are just altered at the point of entry or whenever someone wants alter something.

Hacking to change a result is extremely difficult - you need to know to the rate of votes breaking during the count - match with exit polls. The margins to not arise suspicion - nigh impossible in highly scrutinised swing states.
