No it wasn't hacked. The inappropriate video was made by the creator and did it on purpose for "clickbait" and views. He was using incest and child pornography as a way of gaining views. Why can verified channels do this and not get punished?

The user the video, likely before it could be reviewed. I would have to ask to figure out if anything happened to them, and even then, I doubt they would tell me as it's private info between the user and YouTube. I would be surprised if they didn't issue a strike retroactively.
