It looks horrible, but here's what it means; the hospital failed to send Anthem the doctor's notes, test results and so on that they need to complete the billing. Don't do a thing, the hospital wants to get paid BY ANTHEM and they'll straighten it out themselves.

What’s the assurance the hospital won’t end up billing the patient for whatever the insurance won’t cover? The point for me is that having insurance doesn’t guarantee adequate coverage, let alone coverage that allocates premium & other cost-sharing money as efficiently as Med4All
