Very interesting summation of it! Interesting points! I remember being told that the strange proportions were in part because of the perspective of the viewer out in the piazza.
Pretty sure he just made those proportions to convey he was a boy 12 - 14 or so, as everyone in Michelangelo was muscular, even the women.
No, the David is designedly out of proportion; left hand for example is smaller than the right. Adductor muscles in Michelangelo's paintings are usually tense.
It's not clear how much of that was intentional and how much was due to the marble having flaws in it that forced him to omit chiselling certain parts too deeply.
