Surgical masks do NOT significantly protect you from nanoscale particles! Get a P100 mask and be careful of contacting mucus membranes including your eyes. The GVS Elipse P100 Half Mask is the model I use.

Do travel restrictions contain pandemics or not? Case study: the Black Death. Travel between the steppe hordes and Russian principalities was forbidden. Despite being neighbors, the Black Death didn’t jump between them for *5 years* - it had to pass through Europe first.

Since CDC and FDA haven’t authorized public health or hospital labs to run the tests, right now is the only place that can. So, screening has to be rationed. Our ability to detect secondary spread among people not directly tied to China travel is greatly limited.

We also should expand triggers for screening to detect outbreaks early. Anyone with atypical viral pneumonia where multiplex respiratory viral panel is negative should probably be checked for ; not just those returning from China. But we don’t have screening capacity now

THREAD: A brief review of Catch-22 when it comes to and need for more diagnostic screening capability. Hospitals can roll out RT-PCR based test CDC developed. They all have Roche systems to run these tests. The technology is fairly straightforward. Here’s the rub 1/9

Lancet articles are disturbing even though based upon limited information. One sample saw 29% with ARDS. Terrifying. If true we are not prepared.

For those of you still worried that this is all overblown. 1. I hope you are right. 2. China has implemented the largest quarantine in the history of the planet for some reason. Most major cities are shut. They have the data.

I would like to discuss a strategy for creating such a vaccine with the right organizations. has expertise in peptide-based gene delivery systems which is highly relevant for this particular application and will also prepare us for the future.

We should be looking at combined reactive and prophylactic vaccines as accelerated clinical trials for those whom are severely ill, and then using the prophylactic component of the vaccine on healthy populations after demonstrating safety and efficacy.

Whether or not the virus becomes a US pandemic, we should be treating all steps of preparation — from prophylaxis to therapeutics — as if it were. This will allow the world to rapidly mobilize in the eventual case where a more lethal virus appears.

P100 and N95 masks should be freely available in every city and implemented proactively for public transport, taxis, Ubers, Lyfts and the like.

Additionally, we need to be thinking about Uber/Lyft drivers, bartenders, and the like.

By not allowing second and third hand contacts of hyper-networkers to get free testing, we are opening up an exponential propagation and not doing enough by blocking travel out of China at this stage. This is an opportune moment for the startup ecosystem to shine.
