it's almost always because they've been bitten by ASCII text that someone else passed through MS Word, which then broke for them.

maybe we should kill the phrase "smart quotes" (which I think was the MS trademarked feature?) in favor of "typographical quotes"?

What I don’t like about it is that it implies good typography is extra and optional, rather than default.

I don't think good typography is extra and optional... but like correct grammar and spelling, it might be primarily a human's job.
