I had this happen to me at work. A colleague who shall remain nameless, was doing data entry to a large excel sheet for auditing our stores. Each line was painstakingly checked and reviewed, and then the file saved - on top of the old file. With no backups.

The chair arm rest of the data entry machine was just at the right height that if you leaned too close to the table, it would hit the keyboard - and erase a bunch of lines from the excel. Like hundreds, thousands of lines. And we did not notice this. Until it was too late.
