Mr and Dr would definitely always have been capitalized — maybe that's the secret. You already are in a shift mode so having an r + . is actually labor saving

The "M" and "D" would be capitalized, and then followed by a capital first letter of the name after space, so it sounds like it was actually pretty efficient, otherwise you'd need to switch case twice
