I was in the Army and there is plenty of penalty for failure. Our mission has changed in Afghanistan from holding to offense.

There's no institution-wide penalty. If there were, then a General would be too embarrassed to even contemplate this request.

In officer ranks that usually is a career ender. NCO can possibly come back from that though depends on job.
