Jerry Adler (who played the loanshark Hesh) recalled filming his final scene with James Gandolfini. Afterwards, Gandolfini asked despondently: "I am so tired... will this ever be over?" Doesn't seem like much of a stretch to infer that the character Tony literally killed James.

Also, Edie Falco had almost no personal relationship with Gandolfini. They poured it all into their on-screen relationship, which felt intensely real, but real in a sense that straddled the border between fiction and reality. It's really interesting to hear her describe it.
