Jerry Adler (who played the loanshark Hesh) recalled filming his final scene with James Gandolfini. Afterwards, Gandolfini asked despondently: "I am so tired... will this ever be over?" Doesn't seem like much of a stretch to infer that the character Tony literally killed James.

Yeah, it's rough. Also rough to hear Jerry Adler tell it, he's almost 90 years old and obviously found it painful.
