Why does the app not update in the background? It's completely unusable for me if I can only ever get the new set of emails after already switching to the app.

This isn't expected! Can you please email support@mimestream.com and we'll help figure out what's going on? Please open the Terminal app, and run:
/Applications/Mimestream.app/Contents/Resources/collectDiagnostics.sh
and attach the output to your email.
