          Harder to centrally manage, yes. I’m thinking about chronically ill, economically low productivity people. I don’t have a good answer for them

          Reminder: the U.S. healthcare system is nothing like a free market. Prices are not public, regulation is very heavy, large swathes of the population are covered by "single payer" public entities. The VA system is a parallel public healthcare universe.
