      Want to blow a straight person’s mind? There is a drug called Truvada that a) makes nearly all HIV+ people completely asymptomatic b) prevents them from transmitting HIV c) is a 100% effective prophylactic for HIV d) is available to people with means e) is not for everyone else

      it's absolutely available to people "without means." not only is it covered by most insurance, including medicare, gilead gives it away *for free* to people who can't afford it.

      Insurance requires means. Gilead has given 2.4M bottles to CDC for the 1.1M Americans living with HIV (and the ~359M who don’t have it but could contract it.) They offer a discount to low income people. The discount must be applied for by a doctor. Doctors cost money.

      so we are talking about ppl who can't afford a *single* trip to the doctor for a free prescription to a life-saving drug. seems like an edge case. regardless, if you're that poor, you qualify for medicaid. ppl aren't being infected with HIV bc they can't afford this drug.

      Let me introduce you to some of my friends who borrow money for Truvada. Maybe I know the only two people in the country who do this, but I suspect not.

      i obviously can't speak to the complexities of this particular anecdote. the reality is you can see a doctor in this country if you're too poor to afford one, and gilead offers this drug for free to the poorest americans. targeting cost of drugs alone is not enough to end HIV.

          I agree cost won’t do it alone - awareness and willingness are the other factors, and high-risk opioid users often also have mental health problems that makes this more difficult.

          I’m also unsure if Gilead covers Truvada as a prophylactic. In order to eradicate the disease quickly, we’d want high risk individuals on PreP.

          Lee's still not wrong-- 100 new HIV cases each day in the U.S. -- but that feels like an advocation/education problem more than a resource constraint.
