These can be treated w/o surgery but that’s usually reserved for older, more sedentary people or people w/ risk factors that preclude surgery. Active people usually get it fixed b/c of better long-term strength w/ repair. Minimally invasive tech. has reduced wound complications.

This article on Pubmed is pretty good. Surgical intervention carries a reinjury rate up to two orders of magnitude lower than non-surgical. https://www.ncbi.nlm.nih.gov/books/NBK430844/#:~:text=Treatment%20%2F%20Management,-Operative%20Versus%20Nonoperative&text=The%20initial%20management%20of%20Achilles,versus%20risks%20of%20surgical%20intervention ….

It's strange. The prognosis isn't good, yet there are exceptions. All the orthopedic docs I worked with laughed at the Bo Jackson hip replacement, which was done with an antiquated system. All expected it to fail due to the stress he put on it.

Apparently non-US docs recommend no surgery, US typically cuts. My surgery was straightforward, took a full year before I couldn't tell anything was different other than my hiking boots needed to be bigger. Still run.

I’ve fixed a bunch. Assuming no complications, outcomes typically good to excellent. Typically takes a year to fully recover, but jogging at 4 months post op. I usually recommend surgery in the active population because a small percentage of non-ops heal elongated with poor fxn.
