That rule, invented by typographers to protect smaller characters from breaking off at the ends of lines, makes perfect sense on the internet, where characters with a small number of pixels are in danger of breaking near the dangerous \r\n.

Depends greatly on where you live. Most US style guides have punctuation inside of quotation marks. In the UK is different.
