IMO college campuses already provide a great model: have wide concrete / paved paths that can be used by important vehicles (ambulances, box trucks, etc.) but where it's made clear that they're not intended for use by personal vehicles.

Amsterdam seems intentionally optimized for bikes, with skinny roads for cars, and fat bike lanes. The primacy of the bike lanes is obvious, and everyone bikes everywhere there. The city is also arranged radially.
