In the proof of "a matrix is unitary iff it preserves length", should |ψ> be |e_j> + i|e_k> rather than |e_j> - |e_k>? At first glance it doesn't look like it'd be possible to have the middle two terms in the expansion of ||M|ψ>||^2 have opposite signs if the amplitudes are real.
