Do the people who run restaurants know their websites are bad and don't care or do they really just ... not know?

I think it's usually the second one. Just because they know how to make good food doesn't mean they know anything about graphic design, and they might not have enough money to hire someone who does.
It's less about food vs design as it is running a business that is all about hospitality and failing at that.
