only going eastward, only over the atlantic. I think it has to do with altitude and how far into the flight we reach that altitude, and possibly when i've last eaten but i'm not 100% sure.

International flights tend to have higher altitudes so this makes sense. Have you ever crossed the Rockies, Denver going west? That’s the only national comparable flight that is as bad imo, and one I can’t handle at all thanks to POTS.

yup! zero problem going west. Only a trans-atlantic issue, and only going the one way -- never the same problem coming back.

I “fall asleep” on take off a lot. Especially if tired, and not having eaten in a while. I presume drop in oxygen while pressure stabilizes.
