The whole model is that there’s an order of magnitude difference in individual consumers’ willingness/ability to pay, so charging each consumer different prices raises total revs and increases the amount produced.

Understanding this is fundamental to understanding the healthcare industry. The cost side is actually more broken than the revenue side, and jacking up prices for foreign single-payer programs is a net win even if fundamental changes are still needed.
