I think the airlines have a financial incentive to err on the side of rejecting customers, to not get stuck with ret ticket bill.

Depends on airport. Many international airports have US immigration screening people before they board.

that's already something they've doing for a while - flying to the US w/any visa, ticketing checks it before letting you board.

indeed, even when I fly into the UK or Schengen, I have my passport checked for visas.
