In Chrome, the user can explicitly dismiss the dialog without hitting either of the buttons. Not opposed to the timeout idea either.

For the "dismissal" case, rejection seems appropriate since the user cannot accept after that moment. Timing out raises concerns for reacting to delayed responses. If the UA rejects the promise on its own, it should also close the dialog.

Firefox did this for permissions at one point where the permission prompt was moved to the URL bar. It was awful for developers trying to build a meaningful UI.
