    Today marks exactly 25 years since I lost my father to cancer. Since it was his illness that catalyzed my career in oncology, I’m reflecting on what’s changed a quarter-century later, and what’s remained the same about this wretched disease. THREAD 1/

    On 💡we really need to support more & engage in 🧬biomarker driven . Advances for many of these cancers is coming for subsets. Cancers are heterogeneous👇🏼. One size fits all approach less likely to be successful.

    This is the kind of thread you are lucky to get. Someone with deep expertise taking you through their thought process. Typically, this is reserved for the apprenticeship of academic med, but now, many can learn! (I agree with 97% :) )

    When I first entered medicine I was naively surprised to find it to be a secular discipline. But the more I’ve practiced (especially in oncology) the more I’ve realized how much faith sustains me (& many of my patients) such that I increasingly acknowledge it, when appropriate.

    Remember that most end of life care is provided by: - District Nurses - GPs - General Hospitals - Geriatricians Specialist palliative care is great, but only a certain proportion of people need or get it. Maybe we need to invest in the generalists?

    I am a patient. I lost my father to cancer and now my son has inherited our hereditary tumor syndrome. I will never stop learning about this disease in the hopes of enabling a brighter future for my boy and all those under my care in my oncology practice.

    I know Mr. Limbaugh is, to say the least, a polarizing figure in political media but his case is proof yet again that there is no room for partisanship in our efforts against cancer. Malignancy does not care about party affiliation and none of us are immune to this disease.

    Huge shout-out to my wife for lending me her 4-wheel-drive car and helping clear the driveway so I could eventually make it to clinic today in wintery SLC. As I learned during my fellowship in Minnesota there are no snow days in oncology! Darling ...

    Recalling the day of my son’s birth will always be tinged with the memories of grief and guilt as I rushed from the bedside of a dying patient to his joyful delivery. Such is the balancing act when we walk the tightrope between endings and beginnings.
