is it possible that connecting a USB2 device over a USB3 port would cause issues? It's either that, or because it's being passed into a VM..

very possible. there are tons of protocol translation shenanigans in USB, and they tend to break in obnoxious ways.

Oh never mind. It's just USB forwarding under virt-manager being bad. Works fine in VirtualBox (as much as I'd rather avoid using that)

USB forwarding is always a mess. I wrote the original version of the qemu xHCI passthrough driver (which at the time beat every other system) but it was a fork and I'm not sure what eventually ended up upstream...

You may want to try different settings for the guest host controller (UHCI vs EHCI vs xHCI) and see if any of them works better.
