Getting really frustrated with the Mac boot process. Has someone actually documented this properly, or do I really need to play a random guesswork game until I stumble upon something that works?

The "efi-boot-device" variable is used to set the boot device, as done by bless(8). "efi-boot-device-data" uses the same thing, but encodes it as a EFI_DEVICE_PATH binary path. I think the "Boot####" variables are respected, but it's been a very long time since I've used those.
Can I hold option and pick a FAT32 ESP volume regardless of nvram settings, as long as it has a bootx64.efi?

Yeah. It doesn't even have to be an ESP either. It can just be a single partition device with your EFI booter in one of those locations.
