Does anyone know of any good write-ups on the ins and outs of TPMs? How is firmware updated? Who holds the keys? How do specific vendors control the code that runs on them? Do they get source and modify it? Binary blobs and config?

I don't know anything about the firmwares. My understanding is that TPM vendors provide blobs and updaters and OEMs don't customize anything, but I'm not sure.

Yeah - updates for Lenovo TPMs are certainly wrapped in a Lenovo updater, but not sure about the TPM update itself.

There is a tiny bit of info in this paper: Work Package 5: TPM And "UEFI Secure Boot" Analysis - BSI. It is mostly about win10 interaction with TPM. Says something about some keys being set in hw though.
