      either 1) mouse/keyboard that supports both protocols or 2) an active adapter that has an USB host/PS/2 device inside

      I don't think the latter exist. Even today, USB mice/keyboards very often are still PS/2 compatible. Back in the day it would've been a hilarious waste of money to make active converters like that.

      I'm really curious about what's inside one of those. Did they ever make microcontrollers with a USB host back then? Dedicated IC? How much of USB does it implement?

          I think the first big one is 2001, the Cypress EZ-USB. Despite the part number (CY7C68xxx), it's an 8051, which will never, ever die.

          By 2000 there was at least one cheap chip with a low bandwidth USB 1.x serial interface configurable between host & device signalling. The SL811, for unknown reasons still made by Cypress Semi. Do not look at the datasheet if you value your sanity. (😱🥴😵🤯 You were warned.)

          Wasn't too long ago, maybe 2007 or so? Think it was an FTDI chip, worked with USB keyboards and mice... probably some microcontroller with usb host...

          There were a few projects like that, to convert old Sun keyboards to work with USB, or to use usb/ps2 keyboards on old Sun workstations. I forget which, but possibly both.
