My guess: The MG2040 was made to be a drop-in replacement for the ESD7008 in HDMI applications and allowing slop that doesn't matter for USB/Thunderbolt use since HDMI doesn't need such high bandwidth in the reverse direction.
Everything you mentioned can be explained by the fact that one pinout diagram puts GNDs through diodes (connected to what would be "IO" pins in the other), and the other one hooks up GND directly to the common line of the diodes.
(except the pentagonal pads, but that doesn't mean it isn't the same die and bond-out)
Oh, that's interesting. I suppose that makes sense, driving it one way would provide a more stable signal and different current bias (they're diodes, derp me, of course) than driving it the other way.
