And I can't even prove that that's what happened or repro it; that's just the only logical explanation I could come up with after 1 hour of running tests and reading libvirt code.

It's not a Ceph bug; I've actually never had a bad Ceph bug so far. It's just a libvirt bug/bad default that happens to be triggered more easily by using Ceph.
